https://www.hindawi.com/journals/crig/2016/3056053/

"Kleefstra syndrome is characterized by hypotonia, developmental delay, dysmorphic features, congenital heart defects, and so forth. It is caused by 9q34.3 microdeletions or EHMT1 mutations. Herein a 20-month-old girl with Kleefstra syndrome, due to a de novo subterminal deletion, is described. She exhibits a rare and complex cardiopathy, encompassing multiple coronary artery microfistulas, VSD/ASD, and PFO "

Kleefstra syndrome is a disorder that involves many parts of the body. Characteristic features of Kleefstra syndrome include developmental delay and intellectual disability, severely limited or absent speech, and weak muscle tone (hypotonia). Affected individuals also have an unusually small head size (microcephaly) and a wide, short skull (brachycephaly). Distinctive facial features include eyebrows that grow together in the middle (synophrys), widely spaced eyes (hypertelorism), a sunken appearance of the middle of the face (midface hypoplasia), nostrils that open to the front rather than downward (anteverted nares), a protruding jaw (prognathism), rolled out (everted) lips, and a large tongue (macroglossia). Affected individuals may have a high birth weight and childhood obesity.

People with Kleefstra syndrome may also have structural brain abnormalities, congenital heart defects, genitourinary abnormalities, seizures, and a tendency to develop severe respiratory infections. During childhood they may exhibit features of autism or related developmental disorders affecting communication and social interaction. In adolescence, they may develop a general loss of interest and enthusiasm (apathy) or unresponsiveness (catatonia).

We often wonder how we came to be parents of a child with a disability. It certainly isnʼt what any parent would hope for their child. It has changed our definitions of happiness, success, and family. Living with a child with Kleefstra syndrome has meant living life at a slower pace. We work harder to achieve milestones and appreciate them more when they arrive. Fortunately, we have had a lot of help from family, friends, doctors, therapists and teachers. Because of the rarity of this diagnosis, and the little it has been studied, we arenʼt sure what the future holds for our daughter. We do know she is beautiful, charming, resilient and funny. We are very lucky to have her in our life.

Early Intervention

Children diagnosed with Ks benefit greatly from early intervention consisting of physical therapy, occupational therapy, and speech therapy.

Virtually all children with Ks have low muscle tone. Physical therapy provides support for a child with gross motor delays. Therapists will assist the child in building gross motor skills such as rolling, crawling, walking, and climbing stairs.

Occupational therapy will help a child develop fine motor skills. This can also include sensory-integration, feeding, and self help skills.

Speech delays are another outstanding characteristic of Ks. Early intervention for speech is critical for these children to begin communicating. Speech therapy can often help with feeding issues also.

Other forms of therapy that have been successful and enjoyable for some children are hippotherapy (horseback riding), music therapy and water therapy. Some families have tried conductive education with positive results.

β

Families of affected individuals may benefit from the following forms of interventions, all of which can be practised in the home without the assistance of professional therapists and at little or no cost to the family.β

β

There isnʼt a one-size-fits-all description of Ks because there is a wide range of symptoms and an even wider range in the severity of those symptoms. The variation may be due in part to the number of damaged or deleted genes in the 9q34.3 region, but those with roughly the same deletion size can also have quite different symptoms.
